Voluntary participation
I am at least 18 years old and voluntarily request a complimentary, water-transfer temporary tattoo from representatives of 1221 Creative Studio LLC at LA Comic Con 2026. I may apply the tattoo myself or ask a staff member to apply it. This consent applies to either method.
Risks and skin condition
I understand that temporary tattoos and the application process may cause skin irritation, redness, itching, allergic reactions, or other unexpected skin reactions. I confirm that the application area has no open wounds, rash, sunburn, or visible irritation. I will consider any known skin sensitivity or allergy before applying the tattoo myself, and I will tell the staff about it before any staff-assisted application. I understand that a reaction may occur even if I have not reacted to similar products before.
Application and aftercare
If a staff member applies my tattoo, I consent to the use of water and disposable application materials. If I apply it myself, I will use clean materials and follow the instructions. I will follow the product manufacturer's instructions and warnings, avoid use near eyes or on broken or irritated skin, and remove the tattoo and seek appropriate medical advice if irritation develops. I understand that staff are not providing medical advice or treatment.
Release of liability
To the fullest extent permitted by California law, I voluntarily assume the ordinary risks described above and release 1221 Creative Studio LLC and its owners, staff, and representatives from claims arising from their ordinary negligence in the application of the temporary tattoo. This release does not waive claims that cannot legally be waived, including claims arising from gross negligence, reckless or intentional misconduct, or any nonwaivable statutory or product-liability rights.
Choice to decline
I understand that I may decline the application at any time before it begins. I have had the opportunity to ask questions, and I have read and understood this form.
